Somnia Anesthesia is seeking an Anesthesia Residency Program Director at Dignity St. Joseph Medical Center in Stockton, CA. St. Joseph Medical Center is a tertiary care hospital providing a wide range of medical and surgical services to the surrounding communities. The ACGME accredited Anesthesia Residency program launched in July 2021 with 6 PGY1 and 4 PGY2/CA1 Residents and is on the path for success with positive early results and Resident surveys. The program is approved for an additional 6 Residents annually up to a total of 24 Residents in the program. Residency subspecialty training is currently available at St. Joseph’s include Cardiac, Obstetrics, Critical Care, Acute and Chronic Pain, and eventually, Pediatrics. Practice Demographics: • St. Joseph Medical Center in Stockton, CA is a 355-bed hospital and part of the CommonSpirit/ Dignity health system. • St. Joseph’s is centrally and conveniently located only 50 minutes from Sacramento, San Francisco only 90 mins away, and Yosemite National Park and Lake Tahoe only a few hours away. • The St. Joseph Medical staff tend to reside in the nearby communities of Stockton (Brookside) and the nearby surrounding communities of Lodi, Elk Grove, Manteca, and Tracy. • St. Joseph’s is the largest hospital in San Joaquin County and considered the 10th busiest cardiothoracic department in California. • The hospital has strategic plans underway for a major building and capital services expansion in the next few years. • The Anesthesiology department consists of 10 physicians and over 20 independent-practice CRNAs providing services for up to 20 anesthetizing locations daily to include acute care O.R.s, Cardiac O.R.s, Obstetrics, GI/Endo, Cath Lab, Interventional Radiology, a hospital Ambulatory Surgery Center, and a new 2022 venture with a local Orthopedics Surgery Center. • The annual anesthesia case volume is approx. 15,000 cases consisting of ~10,000 surgical cases, 2,500 Obstetrics cases, 800 Cardiac cases, and 2,000 out-of-OR cases in GI, Cath Lab and Interventional Radiology. Position Description: The Program Director of the Anesthesiology Residency Program must possess the following minimum qualifications and those stipulated in the Accreditation Council for Graduate Medical Education ACGME basic document criteria for program directors: a. Membership in the ACGME. b. Certification by the ACGME through the ABA and recertified within the prescribed time frame of the certifying body. c. Be credentialed and have staff privileges by the department or section of Anesthesia at the base institution. d. Practice of anesthesiology for a minimum of five (5) years or three (3) years full- time with an anesthesiology residency program. Active staff membership within the department of anesthesiology. e. Active staff membership within the department of anesthesiology. f. Fulfill the qualifications as a faculty member of an anesthesiology residency program, including program administration, demonstrated leadership skills, faculty development, anesthesiology training skills, and completion of ACGME continuing medical education (CME) requirements. ACGME requires 150 hours of CME credits over a three-year period, fifty (50) of which are to be in the specialty of anesthesia. g. Active participation in community and professional organizations (i.e.: City, County, State). h. Involvement in research and academic pursuits; Examples may include but are not limited to publication in peer review journals, textbooks, local or specialty publications, formal lectures, and visiting professorships. Program Director Duties and Responsibilities: The program director shall have the following responsibilities: a. Direction of the anesthesiology residency program to ensure that the resident receives the training outlined in the program description. b. Arrangement of formal affiliation agreements and/or outside rotations necessary to meet the basic standards or enhance training to meet the program objectives. Evaluations from the individual responsible at the affiliated training site must be included in the program director’s annual report of each resident. c. The Program Director shall provide evidence of progressive and increasing supervised responsibility by the resident as his/her training progresses. d. The program director must submit quarterly reports to the DME and the ACGME. Annual program director’s reports must be submitted to the ACGME at the completion of each program director’s reports must be submitted to the ACGME at the completion of each year of training. e. The program director shall provide evidence of cooperative assistance in the training of the anesthesiology residents by other departments. f. The program director must provide a printed format of reading assignments for the residents. g. The program director shall schedule formal journal club meetings. h. The program director shall provide evidence of the utilization of allopathic concepts and philosophy in the residency program. i. The program director shall schedule/participate in medical audits, mortality reviews, tissue and tumor conferences. j. The program director shall work with the DME to support pre-doctoral and post-doctoral education and training at the institution. k. The program director must notify the ACGME of all residents in the training program on an annual basis and immediately notify the ACGME preceding a change in residency status. The program director must participate in the annual ACGME Program Director’s Workshop. Attendance is mandatory for the program director, or their designee, to attend a minimum of one Program Director’s Workshop every other year. l. The program director must advise the ACGME’s Evaluating Committee, in writing, with the reason for the resident’s inability to participate in the annual ACGME In-Service Examination within ten (10) days before the examination day.   TITLE:                                Regional Practice Administrator     DEPARTMENT:                                                  Operations   REPORTS TO:                           Director of Practice Management   POSITION DESCRIPTION: Somnia is actively seeking an experienced Regional Practice Administrator with five plus years of experience in either a hospital or large facility setting to work with St. John’s Regional Medical Center in Oxnard, CA and St. John’s Hospital Camarillo in Camarillo, CA. This role is a member of our leadership team and works directly with the Director of Operations. The Regional Practice Administrator will partner with Somnia’s executive management team as well as local hospital leadership. Strong interpersonal, organizational and time management skills are a must. This role will maintain its “home based” facility position while assisting the Director of Practice Management in the management of on-site Practice Administrators, remote support staff, and Manager(s) of Operations in the East Division. In addition, the position will provide project management support for new accounts and ensure proper administrative office set up prior to go-live. REGIONAL RESPONSIBILITIES / TASKS: - Works with Director of Practice Management and corporate leadership team to manage hospital accounts and ensure hospital C-Suite relationships are maintained. Assists with assigned projects needing oversight and coordination of delegation of duties and tasks, reminding participants of deliverables and deadlines, assisting with site visit quarterly report creation, organizing of and preparation for meetings, preparation of agendas and recording of minutes. - Notifies the account VP when they become aware of any issues impacting compliance, operations, and/or billing to include the inability of the practice to meet the contractual service and staffing requirements and delays or issues with billing and the revenue cycle impacting financials. - Ensures responsible facilities P4P is conducted on a quarterly bases and reports/ invoices/ payments occur in a timely manner. - Assists Director of Practice Management in the supervision of Hospital Administrative Staff in dedicated Division.   - Staff Management Oversight - Assists with workflow analysis and ensures all staff have tools needed to execute job expectations - Provides mentorship and guidance for staff - Ensures all staff are adequately educated and IT/equipment/remote accesses are obtained to meet expectations - Assists with recruitment, hiring, termination and remediation of staff in collaboration with Director of Practice Management and corporate HR - Assists with maintenance of pertinent training materials and oversees development of standardized processes and protocols across hospital network. - Works with staff members to execute all projects and quarterly reporting Project Management - Collaborates with Director of Practice Management on all new site implementations - Hospital relationship management with key stakeholders prior to start - Direct administrative resources to assist with project management and new site support - Collaborates/coordinates with Recruitment Department to ensure and conduct new Administrator and on-site support staff hiring, on-boarding, and training - Ensures development of new-hire orientation programs for all on-site clinical and non-clinical staff. - Ensure proper office set up including - IT resources and access - Staff management (Schedule, payroll, credentialing, onboarding, payer enrollment) - Physical plant (office space; call rooms; dept space, etc.) - Remote demographic billing access - Quality Program implementation   HOME BASE RESPONSIBILITIES / TASKS: DEPARTMENT STANDARD: PERFORM DEPARTMENT DUTIES IN A TIMELY AND EFFICIENT MANNER - RCM - Follow standard chart batching and scanning process according to Somnia’s billing partner’s policies and procedures - Work with hospital, Somnia and billing company to scan directly to pertinent imaging system - Submit 100% of charts within 3 business days - Complete or appropriately train support staff to thoroughly scrub charts for all the pertinent pieces of information including but not limited to: - DOS - Provider(s) names / signatures - Start / End times - Accurate and complete procedure / specificity - Diagnosis / specificity / ICD-10 compliant - SCIP measures (temp, beta blocker, sterile technique, etc. even if just for internal audit) - Documentation of who completed any ancillary services (i.e. blocks, lines, etc.) - Service Location (Hospital, ASC) - Sub-Location (i.e. OR1, OB, Endo, etc.) - Ensure VP of Patient Revenue has complete lists of service locations and sub locations - Lead discussions to establish interfaces between the hospital and billing partner to include: - Clinical documentation – only applicable to electronic anesthesia documentation sites - Billing interface - Demographic interface – should be available at all locations - Maintain 100% accurate case log - Information entered in the Admin Database within 3 business days of DOS and updated daily - Reconciliation within 1 week of email notice when discrepancies with billing case log when identified - Assist to ensure enrollment of new clinicians with payers prior to start date including but not limited to: - Follow up to ensure paperwork is sent to new clinician - Fully completed paperwork is returned and received - Send out START DATE NOTIFICATION email no later than 45 days prior to start. ie: paper, smartphone, etc. - Ensure completion of at least 1 cost savings project annually - Formally Document on-going PI projects (not just on HAR spreadsheet) - Ensure TIMELY completion of F/OPPEs - Load on shared drive folder and provide copy to hospital MSO. - Review hospital SCIP data with chief and staff, if applicable - Report HOSPITAL calculated results monthly on HAR - Enter all results in numerator/denominator fashion so sample size is easily identifiable. - If hospital is not tracking and reporting SCIP data complete annual audit of SCIP measures and include in HAR - Ensure OB epidural tracking times are reported and either captured locally or sent to billing partner - Patient Satisfaction: Facilitate and support improvements, especially with OB patient satisfaction, ie: patient education, tablet e-survey roll-out, etc. - Ensure competency with accessing and using the MTC web portal to access data, update practice info, produce reports, etc. - Support roll out and utilization of OB tablet surveys - Ensure safe storage and maintenance of tablet - Conduct annual surgeon satisfaction survey to obtain feedback from > 30% of surgeons (or more frequently if required by contract) - Support annual nursing satisfaction survey, if applicable - Develop, implement and audit action items based on patient, surgeon and nursing satisfaction survey results - Ensure there is an action plan to improve scores and report on this monthly on HAR spreadsheet - Respond timely to requests for information / data collection for creation of quarterly, annual, and ad-hoc client reports - Implement, maintain and make available an up to date policy and procedure manual with: - At least annual review an sign off by chief - Sign off sheet for all credentialed anesthesia clinicians for your facility acknowledging review and understanding of contents - Outcomes Forms – ensure an Outcomes Form is completed in full for each case. - Incident Reporting:  Facilitate and support improvements in reporting rates - Ensure an electronic incident report is completed for each adverse event at the end of the Outcomes Form for that case. - Help identify reporting trends and address suspected under reporting - Facilitate timely chief reviews and processing within guidelines - QM database: Maintain complete, accurate, and timely database entries - Frequent database maintenance - Competent at pulling data and reports as needed for practice and Somnia - Be able to discuss trends in the types of incidents reported. - Facilitate entry of timely chief reviews in the database - Peri-op/OR data:  If the site doesn’t already have readily available and accurate Periop/OR data on the traditional KPI metrics, facilitate and support a process to either implement a new electronic tracking process or one by hardcopy form - ACER: - Support and facilitate compliance with the use of the ACER program (minimum monthly staff access and review) to include electronic and posted reminders for the staff and discussion at staff meetings. - Support and facilitate the use of the data for F/OPPEs and Chief reviews. - Staff Awareness/Education: Support and facilitate the regular presentation and discussion of compliance and quality related data and issues during staff meetings, etc. - Post compliance/quality related data and materials at appropriate locations to for staff reminders and awareness of goals and performance.

Position Summary The Clinical Operations Diplomat, CRNA will work in conjunction with the following team members:  Chief Operations Officer, Chief Medical Officer, VP Clinical Operations, Regional Medical Directors, Regional Practice Administrator, Director of Quality, and local leadership (Facility Medical Director and/or Chief CRNA, On-site Administrator) to provide facility management and clinical services for Somnia, Inc. and affiliates anesthesia facility contracts. Purpose Responsible for the delivery of clinical services to assigned contracts as well as leadership and technical clinical training services.  Position Description Under direction of the COO, the Clinical Operations Diplomat assumes leadership and clinical responsibility for the CRNA care team operations at accounts throughout our national client base. Serves as onsite clinical member participating in call, enhanced clinical training and daily clinical operations. The Clinical Operations Diplomat’s will be involved in scheduling, daily staffing, and flexing staffing to match OR volume. Assisting with care team development, recruitment and supporting the Clinical Operations team goals and objectives. This position collaborates with, and evaluates all assigned shift Associates according to hospital, anesthesia and unit philosophy and objectives. Utilizes and acquires knowledge of current and best practices in the specified clinical and management areas of anesthesia practice. This position is essential as it also provides ongoing clinical support to facilities that are starting up or in need of clinical staff or leadership.  Responsibilities/Tasks - Provide clinical anesthesia in facilities requiring additional staff to meet contracted points-of-service. It is expected this position will provide clinical services on average of five (5) days per week.
